E-cigarette vapour has been reported to contain up to 7×1011 free radicals per puff.10 Both nicotine-free and nicotine-containing condensate induced a significant increase in ROS release from our AMs, which may explain the induction of apoptosis in nicotine-free liquid. There was significantly greater ROS production in AMs treated with nicotine-containing condensate than in nicotine-free condensate, once again suggesting both nicotine dependent and independent mechanisms at work. The effects of ECVC with and without were ameliorated by NAC and PI3K inhibition. A Foundation-led study presented at the 2024 Society for Research on Nicotine and Tobacco annual meeting found that monthly e-cigarette sales in California declined by 41% after statewide restrictions on the sale of flavored tobacco products took effect. However, the report also noted that the tobacco industry continues to market products to offset these declines, including non-menthol cigarettes that contain other cooling chemicals.
At baseline, demographic and smoking history variables were collected, including age, sex, ethnicity, education, marital status, income, health status, age of starting to smoke, cigarettes smoked per day, and previous cessation attempts. Participants also completed the FTCD,15 Hospital Anxiety and Depression Scale, 16 and Chronic Obstructive Pulmonary Disease (COPD) Assessment Test.17 Objective measures included weight, height, blood pressure, heart rate, and expired CO reading. These aerosols can contain potentially harmful substances — including nicotine, formaldehyde and metals — some of which cause cancer and other harmful health effects.
All research was done in a quiet, temperature-controlled room at the University of California Los Angeles (UCLA) Human Physiology Laboratory. Participants were asked to lie down on a recliner with a footrest and 10 electrodes were placed on their chest following standard echocardiogram (EKG) protocol. Blood was drawn for baseline levels of nicotine and cotinine — a metabolite of nicotine — and then the EKG recorded for five minutes while the room was kept quiet.
Respondents also reported keeping or selling the devices, or returning them to a vape shop. In addition, the percentage of EVP use in 2021 was higher in whites (6.5%) than Blacks (3.1%), Asians (1.2%), and Hispanic/Latinos (3.4%) compared to 2015. However, white and Black adolescents had the highest increases of about threefold between 2015 and 2021. In all four survey years, daily EVP use was highest in grade 12 where most students are ages 17 to 18.
Both increased markers of oxidative stress and decreased NO bioavailability, flow-mediated dilation, and vitamin E levels showing no significant differences between tobacco and e-cigarette exposure (reviewed in [20]). Inasmuch, short-term e-cigarette use in healthy smokers resulted in marked impairment of endothelial function and an increase in arterial stiffness (reviewed in [20]). Similar effects on endothelial dysfunction and arterial stiffness were found in animals when they were exposed to e-cigarette vapor either for several days or chronically (reviewed in [20]). In contrast, other studies found acute microvascular endothelial dysfunction, increased oxidative stress and arterial stiffness in smokers after exposure to e-cigarettes with nicotine, but not after e-cigarettes without nicotine (reviewed in [20]). In women smokers, a study found a significant difference in stiffness after smoking just one tobacco cigarette, but not after use of e-cigarettes (reviewed in [20]). One of the first studies in humans involved the analysis of 9 volunteers that consumed e-cigarettes, with or without nicotine, in a ventilated room for 2 h [8].
The inhalation of harmful chemicals can cause irreversible lung damage and lung diseases. This study examined the impact on adult e-cigarette users from the recent e-cigarette-only flavor bans in Washington State, New Jersey, and New York. We found that most respondents continued to use e-cigarettes with banned flavors post-ban and obtain banned-flavor e-cigarettes through legal channels. Enforcing compliance of retailers to the policy is crucial to control e-cigarette use. First, we depicted the characteristics of respondents from the three states with chi-square tests to evaluate demographic differences by state and the distribution of respondents by their tobacco and nicotine use status.

Still, it’s far from safe and can lead to nicotine addiction and potentially fatal lung disease. Electronic cigarettes—or e-cigarettes—are battery-powered devices that convert a liquid, usually with nicotine, into an inhalable aerosol. They are designed as an alternative to smoking traditional cigarettes. First introduced in the United States about 15 years ago, e-cigarettes have evolved from disposable, single-use products to more sophisticated devices that can be refilled and customized (fig. 1). One of the largest studies done on this was published in The New England Journal of Medicine in 2019. The study, which included 886 randomized participants, compared e-cigarettes and nicotine replacement therapy, which included patches, gum, lozenges, nasal spray, and mouth spray.
On April 22, the Plan Commission denied the proposal, based on inadequate parking. Some members expressed concerns about the proximity to schools and the growing number of vape shops. “This business would in essence serve as a tobacco billboard advertisement to those young persons passing by every day on the way to and from school,” Police Chief Brian Chaney wrote in a letter to city staff. The Milwaukee ordinance and the Monona moratorium apply to stores where vaping supplies make up at least 10% of stock or floor space. That is meant to avoid gas stations or other stores that sell some e-cigarettes.

President Trump signed the bill into law on December 20, 2019 and it immediately took effect. As the user draws on the device, the battery heats the e-liquid to produce aerosol (not water vapor). The battery-operated devices come in many forms and can look like conventional cigarettes, pens or even sleek tech gadgets. This way of taking in nicotine poses health risks to both users and non-users. The 2021 report also provides details on some characteristics of e-cigarette products, including flavors and nicotine concentration, as well as the bundling of the components in cartridge systems. The data shows that in 2021, 69.2 percent of e-cigarette cartridges either sold or given away contained menthol-flavored e-liquids, and the rest were tobacco-flavored. The monitoring of other tobacco product sales in conjunction with the monitoring of e-cigarette sales is necessary to provide a comprehensive picture of the overall U.S. tobacco product market.
There are other proven, safe, and effective methods for quitting smoking. One way to start is to talk with your doctor, nurse, or a trained quitline counselor to figure out the best strategies for you. The liquids used in e-cigarettes do not contain tobacco, but they do contain nicotine derived from tobacco, and thus the U.S. Food and Drug Administration (FDA) classifies them as a tobacco product. FDA regulates the manufacture, import, packaging, labeling, advertising, promotion, sale, and distribution of e-cigarettes that meet the definition of a tobacco product. E-cigarettes may also be used to aerosolize other substances for inhalation, including cannabinoid products.
The age restriction (21+) has only created an entrepreneurial opportunity. Kids are buying e-juice, filling cartridges and selling them to others not able to obtain product on their own. Indeed, the “vapor” may have a lot more than those five ingredients listed above. A 2018 study of e-cig smokers’ urine found at least five of the same carcinogens found in cigarettes. Like many states, New Jersey made it illegal to sell vaping products to anyone under 21 and then went a step further by banning flavored vape juice. CDC also identified that most of the student users were influenced by mass media and their peers to use ENDS related products which often resulted in health issues such as delayed brain development, lung damage and the possibility of future addiction to other drugs. While the percent of young people in Massachusetts using traditional combustible cigarettes has fallen, there has been an exponential increase in the percent of young people using e-cigarettes and vaping devices. In 2019, 4.3% of Massachusetts high school students reported current use of combustible cigarettes and 32% reported current use of electronic cigarettes.
Clearly the dangers extend beyond what is being done to the lungs and heart. Adults who used e-cigarettes at least once a week before the flavor bans were recruited online. Respondents reported their e-cigarette use, primarily used flavor, and ways of obtaining e-cigarettes before and after the bans.
The long-term risks of exclusive use of e-cigarettes are not fully known but evidence is accumulating that e-cigarette use has negative effects on the cardiovascular system and lungs. Without immediate measures to stop epidemic use of these products, the long-term adverse health effects will increase. FDA is committed to protecting the public health of the U.S. population while regulating an addictive product that carries health risks. We’re conducting ongoing research on potentially less harmful forms of nicotine delivery for adults, including studies of e-cigarettes and ENDS. Many studies suggest e-cigarettes and noncombustible tobacco products may be less harmful than combustible cigarettes. However, there is not yet enough evidence to support claims that e-cigarettes and other ENDS are effective tools for quitting smoking.

There is an urgent, overdue need for standardized processes for the disposal of e-cigarette devices, refills and e-liquids. Waste management and hazardous waste disposal plants are not currently equipped to handle e-cigarette waste and federal regulations still have not caught up to the need for guidance on disposal of these types of small electronics containing toxic waste. Federal, state, and/or local governments need to set clear standards on environmentally responsible e-cigarette waste disposal and hold the industry accountable for adhering to them. E-cigarettes not only pose substantial health risks to youth and young adults, they pose a significant environmental threat (see the Truth Initiative fact sheet on Tobacco and the Environment).
The nicotine content of the liquid can range from “very high” to zero. “EVP use is not a safer alternative to smoking but may have contributed to the decline in regular tobacco product use. Endothelial cell migration is an important mechanism of vascular repair than can be disrupted in smokers due to endothelial dysfunction [25, 26]. The decision to pursue a smoking cessation objective, even in such a controlled form, should be made only after considering national circumstances, along with the risk of uptake and after exhausting other proven cessation strategies. While the Family Smoking Prevention and Tobacco Control Act of 2009 prohibited the sale of most flavored cigarettes, the flavor ban did not extend to menthol flavored cigarettes or to any flavored non-cigarette tobacco products such as cigars, hookahs, and e-cigarettes.
We show a significant increase in cytotoxicity caused by the vaping process itself. E-cigarettes are called many different names, including “vapes,” “e-cigs,” “vapor products,” and “electronic nicotine delivery systems (ENDS).” At the same time, there have been concerns that e-cigarette flavor bans might prompt some smokers or former smokers using e-cigarettes to return to exclusive smoking. But the actual impact of flavor restrictions at the state or local level on adult and youth tobacco use patterns is unclear. In the present review, we have attempted to clarify these questions based on the existing scientific literature, and we have compiled new insights related with the toxicity derived from the use of these devices. The e-liquid typically contains humectants and flavourings, with or without nicotine; once vapourised by the atomiser, the aerosol (vapour) provides a sensation similar to tobacco smoking, but purportedly without harmful effects [3]. However, it has been reported that the heating process can lead to the generation of new decomposition compounds that may be hazardous [4, 5]. The levels of nicotine, which is the key addictive component of tobacco, can also vary between the commercially available e-liquids, and even nicotine-free options are available. For this particular reason, e-cigarettes are often viewed as a smoking cessation tool, given that those with nicotine can prevent smoking craving, yet this idea has not been fully demonstrated [2, 6, 7]. While fewer people are smoking or starting to smoke than ever before, many are using other forms of tobacco and electronic nicotine delivery systems.

The agency has taken action on over 96% of the applications to date, including issuing marketing denial orders (MDOs) for more than one million flavored ENDS products that are so popular with young people. The MDOs were issued for products whose applications lacked sufficient evidence that such products have a benefit to adult smokers to overcome the public health concern posed by the well-documented and considerable appeal of the products to youth. The FDA is aware of a number of companies, such as Puff Bar, claiming their products contain only synthetic nicotine not sourced from tobacco, which may raise separate regulatory and legal issues that the agency is considering how best to address.
For example, tobacco-flavored e-cigarette cartridges sales dropped from 47.2% of those sold or given away in 2015 to 21% in 2018, while the sale of “other” flavored cartridges tripled in that time, increasing from 13.8% in 2015 to 42.1% in 2018. This was significant because Chantix, at one point, had a “black box warning” after reports linking the drug to psychiatric side effects. That warning was dropped in 2016 after a very large study showed the drug to be safe, but Toll and Fucito said the stigma of the warning remains in the minds of both health care providers and the general public. The American Association of Poison Control Centers, reports as of May 31, 2022, poison control centers have managed 2,451 exposure cases about e-cigarette devices and liquid nicotne in 2022. Design, Setting, and Participants This cross-sectional study used data from the 2021 Behavioral Risk Factor Surveillance System (BRFSS) database. The BRFSS is the largest national telephone-based survey of randomly sampled adults in the US. Adults aged 18 years or older, residing in 49 US states (all except Florida), the District of Columbia, and 3 US territories (Guam, Puerto Rico, and the US Virgin Islands), were included in the data set. Nicotine can harm brains as they develop—which continues until age 25 or older!
In fact, the overall evidence points to e-cigarettes actually helping people to give up smoking tobacco. There is no evidence that e-cigarettes are undermining England’s falling smoking rates. Instead the evidence consistently finds that e-cigarettes are another tool for stopping smoking and in my view smokers should try vaping and vapers should stop smoking entirely. Emerging evidence suggests some of the highest successful quit rates are now seen among smokers who use an e-cigarette and also receive additional support from their local stop smoking services. Marijuana-containing vape products contain concentrated forms of the plant, which means THC levels are many times higher than in the highest-quality marijuana.
A 2018 Truth Initiative survey found that mint was among the top three favorite flavors among young JUUL users aged 12-24, meaning they chose it last time they vaped. New research shows that mint and menthol e-cigarette use among high school users rose from 16% in 2016 to 57.3% in 2019. Among high school JUUL users, 67.5% reported that their preferred flavor was mint or menthol. Another study, conducted before JUUL pulled its other flavors from the market, found that mint was one of the most popular flavors among high school student JUUL users, but that menthol was less so.
Principal investigator Wendy Max, PhD lost vape price, director of the Institute for Health & Aging, noted that from 2013 to 2018, e-cigarette use among high school students soared from 4.5% to 20.8%. Protective factors may reduce the risk of youth engaging in substance use. Increasing protective factors in addition to reducing risk factors can be more effective in preventing substance use. Another piece of good news – those who stopped vaping didn’t boomerang back to cigarettes. None of the participants in this study experienced serious side effects, although a larger study would be needed to verify this finding.
