The most recent evidence suggests that e-cigarettes may pose their own unique health harms and that comparison to cigarettes may not be the only relevant question for determining their impact on individual health. Indeed, the growing evidence of potential health risks related to e-cigarette use has led some researchers to question whether e-cigarettes are safer than combustible cigarettes. Electronic cigarettes (e-cigarettes) is the name given to a group of battery-operated tobacco products that allow users to inhale aerosolized liquid (e-juice) containing nicotine and other substances. In addition to its toxicological effects on foetus development, nicotine can disrupt brain development in adolescents and young adults [44,45,46]. Several studies have also suggested that nicotine is potentially carcinogenic (reviewed in [41]), but more work is needed to prove its carcinogenicity independently of the combustion products of tobacco [47]. In this latter regard, no differences were encountered in the frequency of tumour appearance in rats subjected to long-term (2 years) inhalation of nicotine when compared with control rats [48]. Despite the lack of carcinogenicity evidence, it has been reported that nicotine promotes tumour cell survival by decreasing apoptosis and increasing proliferation [49], indicating that it may work as a “tumour enhancer”. In a very recent study, chronic administration of nicotine to mice (1 mg/kg every 3 days for a 60-day period) enhanced brain metastasis by skewing the polarity of M2 microglia, which increases metastatic tumour growth [50].

Indeed, the “vapor” may have a lot more than those five ingredients listed above. Some studies have found it to contain lead, nickel, tin, and silver from the machinery inside the devices along with formaldehyde, manganese, tolulene, and other ingredients linked to cancer, central nervous system problems, and other possible health issues. A 2018 study of e-cig smokers’ urine found at least five of the same carcinogens found in cigarettes. In addition, flavored tobacco products are known to make it difficult to quit nicotine.
Researchers are working hard to gather more information about e-cigarettes and how they are used. This information may lead to additional regulations and could be helpful for informing the public about what’s in e-cigarettes and the potential health risks of using them. Studies on e-cigarettes as a quitting aid for nicotine addiction and cigarette use are few, with mixed and modest results. Some show that they are about equal in effectiveness to that of nicotine replacement therapy (NRT) – like the nicotine patch. The best way to quit is to use quit line counseling and FDA-approved medications. E-cigarette use among both youth and young adults has increased considerably in recent years.

Drawn to the fruity flavor cartridges, or trying to wean from traditional tobacco products, or even to curtail hunger, many people believe e-cigarettes or “vaping” is safer than smoking cigarettes and simply not addictive. As shown in Table 1, the majority of the 1624 respondents were males (61.2%), young adults between 25 and 34 years old (56.9%), Whites (59.4%), and had at least a bachelor’s degree (66.8%). The majority of respondents had used e-cigarettes between two and five years (69.6%) and 79.8% had moderate or strong intentions to quit. The percent of respondents saying they used e-cigarettes because of the added flavors or for quitting smoking cigarettes were 54.4% and 36.6%, respectively.
The health protection agency reports that 2.8 percent of American adults used e-cigarettes in 2017. Among adult e-cig users, 58.8 percent also continued to smoke tobacco cigarettes while 29.8 percent were former tobacco smokers. E-cigarettes hit the market in the early 2000s and have since soared in popularity, especially among teens and young adults. Once believed to be a “safer” way to smoke, vaping is now considered a public health crisis by many health groups. In Sonoma County, 13 percent of 11th graders said they have used an e-cigarette, according to the Department of Health Services.
Many popular e-cigarettes, like JUUL, are pod-based with single-use plastic cartridges containing nicotine. The flavors that are the most popular with kids are fruit, candy, mint, and menthol.
Because e-cigarettes contain nicotine derived from tobacco, they are now subject to government regulation as tobacco products. In December 2019, the federal government raised the legal minimum age of sale of tobacco products from 18 to 21 years, and in January 2020, the FDA issued a policy on the sale of flavored vaping cartridges. Products marketed for therapeutic purposes (for example, marketed as a product to help people quit smoking) are regulated by FDA’s Center for Drug Evaluation and Research (CDER). FDA published a rule clarifying when products made or derived from tobacco are regulated as tobacco products, drugs, and/ or devices.
The long-term risks of exclusive use of e-cigarettes are not fully known but evidence is accumulating that e-cigarette use has negative effects on the cardiovascular system and lungs. Without immediate measures to stop epidemic use of these products, the long-term adverse health effects will increase. FDA is committed to protecting the public health of the U.S. population while regulating an addictive product that carries health risks. We’re conducting ongoing research on potentially less harmful forms of nicotine delivery for adults, including studies of e-cigarettes and ENDS. Many studies suggest e-cigarettes and noncombustible tobacco products may be less harmful than combustible cigarettes. However, there is not yet enough evidence to support claims that e-cigarettes and other ENDS are effective tools for quitting smoking.

ROS production in response to cigarette smoking (or smoke extract) has been implicated as a mediator of adverse effects,23 therefore we examined the possible utility of N-acetyl cysteine (NAC) treatment in reducing the harmful effects of ECVC in THP-1 macrophages. Both AM and THP-1 macrophages were used for these experiments due to the large number of experimental conditions and numbers of cells required.

We show a significant increase in cytotoxicity caused by the vaping process itself. Importantly, exposure of macrophages to ECVC induced many of the same cellular and functional changes in AM function seen in cigarette smokers and patients with COPD. While further research is needed to fully understand the effects of e-cigarette exposure in humans in vivo, we suggest continued caution against the widely held opinion that e-cigarettes are safe. Third, our model represents an acute exposure, rather than a chronic exposure system which is better suited to in vivo animal experiments.

The electronic cigarette (e-cigarette), for many considered as a safe alternative to conventional cigarettes, has revolutionised the tobacco industry in the last decades.
Although COVID-19 affected our findings, its exact association with the prevalence of e-cigarette use is unclear. Future studies should assess pandemic-specific factors, such as lockdown effects on e-cigarette availability and use. Longitudinal data during the pandemic can provide insights into evolving behaviors. Similarly, the proportion of individuals reporting never using combustible cigarettes among those who reported daily e-cigarette use was highest among young adults aged 18 to 20 years (66.5% [95% CI, 61.2%-71.4%]) (Table 4).
